This file is the authoritative public index of temporary constitutional exceptions governed by Article 12 of the Constitution. An entry is not valid merely because it appears here: it also requires the exact A4 approvals, eligibility, evidence, scope, and unexpired term required by the Constitution.
Sensitive evidence belongs in a protected system. The public entry must retain a reader-safe summary and a random, non-derivable attestation identifier that an authorized reviewer can resolve to the complete restricted record. This register never contains secrets, personal data, private Lore, live vulnerability details, or questionable rights material.

Each active or historical entry records:
- stable exception identifier and status;
- Constitution version, immutable revision, article, and exact eligible requirement;
- accountable owner, affected A4 domain owners, constitutional steward, and approval times;
- exact scope covering systems, actions, data, artifacts, audience, destination, repositories, and prohibited uses;
- necessity, alternatives, rationale, risks, compensating controls, monitoring, stop conditions, rollback, remediation, and notification;
- start, expiry of no more than 90 days, renewal lineage, and review schedule;
- public evidence and a safe restricted-record attestation when needed;
- actual use, affected outputs and downstream consumers, incidents, residual obligations, validation, and closure outcome; and
- linked issues, pull requests, amendments, policies, and follow-up work.
Denied, revoked, expired, superseded, and closed entries remain in the historical section. Renewal creates a new linked entry and never edits an expired term into an active one.
