Limit number of times tutorial can be attempted

If a worker fails to pass the tutorial in X times (where X is user-selectable), we will disqualify them from attempting the experiment in the future.

Also: cool down timer on being asked to do the task (if you're a worker and you're never the person that gets to do the task, then we should just stop getting you to come back). For instance, you are the slowest to press "Ok" when the "Routing you to task, are you ready?" alert message, then we'll set a value where if you tried 5x an hour, we'll just stop you from doing the task temporarily.
