Pushing direct access buttons in quick succession causes device to stop responding, and it stops responding to other button inputs. Cycling the power is required to make the device functional again.

One possible cause is an out of bound array index:
play_transition[2] is defined and then called later as play_transition[1] and play_transition[2] rather than play_transition[0] and play_transition[1].
