issue-review/prepare output cross-posts content from an unrelated issue #115

Description

@ellen-goc

Summary

During a 2026-07-12 org-wide review, weyucou/ibuki#3's most recent comment (titled "Development Readiness Assessment", posted ~2026-07-11) contains content entirely about a different, unrelated project — hangboard-trainer's attempts-tab port, expo-video, etc. — not about ibuki#3's actual subject (a conformance-check MCP tool, pillar 5).

Impact

A future readiness/prepare/develop pass on ibuki#3 could be misled by this comment into treating stale hangboard-trainer content as ibuki#3's own readiness state.

Repro context

Not independently reproduced — surfaced via an org-scan review session that ran several parallel askcc sessions across different repos/issues around the same time window. Possible cross-talk between concurrent sessions writing to the wrong issue URL, or a context-bleed bug in whichever askcc action produced that comment.

Ask

Investigate whether concurrent askcc invocations across different repos can cross-post output to the wrong GitHub issue. If confirmed, this is a correctness/isolation bug that could misdirect any comment-based readiness state.
