fix(create): avoid false positives for natural lore vocabulary

Summary

• narrow lore quality checks so ordinary words such as "model", "cost", "framework", and "api" are not incorrectly rejected
• add regression coverage for legitimate lore vocabulary and genuine meta-content leakage
