Design official Patch Pulse favicon / icon

Summary

Design an official Patch Pulse icon/favicon that can be used across all brand touchpoints.

Requirements

• Works at small sizes (16×16 for VS Code file icons, 32×32 favicon)
• Works at larger sizes (128×128+ for extension marketplace, npm, etc.)
• Delivered as SVG so it can be scaled / recoloured as needed
• Should reflect the "pulse" / heartbeat theme of the project

Outputs

• SVG source file committed to assets/
• PNG exports at key sizes (16, 32, 64, 128, 256)
• Favicon added to any docs / landing pages
