Adds a 'Ogmios tip is stale' alert rule that fires when a connected Ogmios instance's chain tip (ogmios_tip_slot) stops advancing for 15m, catching wedged chain-sync sessions that report connectionStatus=connected but serve a stale tip (as seen on v6 mainnet). Notifies #infra-alerts. Adds a matching 'Tip Slot Advancement (changes / 15m)' timeseries panel to the Cardano Ogmios dashboard visualizing the same signal per pod.
