Minor bugfix when using "Title from first bold"

Fixes (title from first bold enabled only):

• Multiple callout blocks in a row would result in lines being lost
• Bold text has to be on the correct indentation level (stricter regex)

v1.17.0 - Title from first bold text option

Adds an option to use the first bold line as a title, this behaviour is disabled by default:

Will render as:
