fix: settings that read as settings in /plugin

The /plugin form showed "Only past this context fill" and "Model that
judges": half sentences that looked like menu entries rather than fields
to type in, with no unit, no default and no word on what an empty field
does. Pressing Enter on them did nothing a person could see.

They are now "Context % before taskcut acts" and "Model that spots task
switches", and each description says first what to enter and that empty
keeps the default, the why after, in two lines at most. The README's
table names the same titles. The keys are unchanged, so saved settings
still apply.
